INSURANCE COVERAGE FOR STAFF AND STUDENTS OF
SIR ARTHUR LEWIS COMMUNITY COLLEGE

PLAN OUTLINE

1. ACCIDENTAL DEATH AND DISMEMBERMENT

If death, dismemberment or loss of sight occurs to an insured because of injury effected solely through accidental means and within ninety (90) days after the accident, payment will be made as listed in the schedule of benefits.

With regard to hand and feet, loss shall mean dismemberment by severance at or above wrist or ankle joints respectively; with regard to eyes, total and irrecoverable loss of sight.

2. ACCIDENTAL MEDICAL EXPENSE BENEFIT

Under this programme benefits will be on a cash reimbursement basis for eligible expenses incurred for medical treatment commencing within ninety (90) days of the date of the accident/injury.

3. TOTAL PERMANENT DISABILITY

When as the result of injury and commencing within one hundred and eighty (180) days after the date of the accident, the named Insured is Totally and Permanently disabled and such disability has continued for a period of twelve (12) consecutive months and is total, continuous and permanent at the end of this period, the insurer will pay the Principal Sum less any other amount paid or payable under this form as a result of the same accident, at the rate of one percent (1%) per month for each month the named Insured remains Totally and Permanently disabled, to a maximum of one hundred (100) months.

4. TERMS AND CONDITIONS

Under this program, one hundred percent (100%) participation is required. A list of the registered member must be submitted to the insurer at the beginning of each policy year.

The applicable amount (as outlined below) will be paid if you suffer a loss listed in the Schedule of Losses which occurs within ninety (90) days of an accident which happens while you are insured.

Loss of one hand and one foot 75,000.00

Loss of one hand and the sight of one eye 75,000.00

Loss of one hand or one foot 37,500.00

Loss of sight of one eye 37,500.00

Total Permanent Disablement other than the above 60,000.00

Medical expenses incurred by accident 15,000.00

STUDENTS

BENEFITS

Accidental Death $20,000.00

Loss of both hands or both feet 20,000.00

Loss of sight of both eyes 20,000.00

Loss of one hand and one foot 20,000.00

Loss of one hand and the sight of one eye 20,000.00

Loss of one hand or one foot 10,000.00

Loss of sight of one eye 10,000.00

Total Permanent Disablement other than the above 20,000.00

Medical expenses incurred by accident 10,000.00

The maximums quoted above are applicable for each person per Policy Year.

“Policy Year” shall mean the twelve month period commencing from the effective date and ending twelve months thereafter.

AD & D

AD & D INSURANCE

Losses Not Covered

No AD & D benefits will be paid for loss caused or contributed to by:

1. Bodily or mental illness, infirmity or disease of any kind, or medical or surgical treatment thereof; or

2. Ptomaines or bacterial infections (except pyogenic infections occurring with and through an accidental cut or wound), or hernia, or medical or surgical treatment thereof; or

3. Attempted suicide or intentional self-destruction; or

4. War or act of war, declared or undeclared; or

5. Voluntary participation in a riot or insurrection, or as the result of the commission of an attempt to commit and assault; or

6. Activity while on duty as a member of any military organisation; or

7. Participation in flying, ballooning, hang-gliding, parachuting, or other aeronautic activities, except as a fare paying passenger on a licensed commercial aircraft which is operated by a licensed pilot on a regular passenger route; or

8. The voluntary use of any drug, narcotic, or hallucinogen, except as prescribed by a licensed physician; or

9. Driving while intoxicated as defined by law.
